Transitional Care

St. James Healthcare's Transitional Care Unit (TCU) is a 12-bed, skilled care unit located on the sixth floor of the hospital. It’s a comfortable setting where patients can wear their own clothes, interact with other patients, and enjoy a measure of independence while recuperating.

When full hospitalization is no longer required, but additional care is still needed, the St. James TCU allows an option for recovery. The unit provides the same level of skilled care and top quality medical services found at St. James in the hospital setting. Healing and recovery are best managed when emphasis is placed on getting patients back home.

A comprehensive team of caring professionals assists recovery. TCU is certified by Medicare to provide patients with specific skilled care services after their acute-care hospital stay. In addition to Medicare, TCU also uses other insurance programs.

Skilled care services provided are:

  • Nursing services for IV medications, skin and wound dressing changes, medication management, and adjustment and teaching activities.
  • Physical therapy to improve and restore patients' abilities to move about in their homes and the community.
  • Occupational therapy to assist patients in resuming self-care abilities.
  • Speech therapy to evaluate and treat speech, language and swallowing problems.
    Skilled services are ordered by physicians and monitored by Medicare for payment approval.

The TCU offers:

  • Bright, cheerful, private and semi-private rooms
