At St. James Healthcare, our pharmacists are an integral part of your healthcare team working closely with physicians and nurses to assure the best pharmaceutical care for our patients. Often the patient does not even realize someone is constantly monitoring their medications for safety, duplications, interactions or adverse reactions. Our physicians and nurses have come to rely on our expertise and we enjoy their trust.
The St. James Healthcare Pharmacy has over 120 years of combined experience and provides comprehensive, distributive, consultative and clinical services 24-hours per day, 7-days per week.
We have a full-time staff of nine pharmacists and four Certified Pharmacy Technicians. Our technicians have all passed the national certification test and complete yearly continuing education programs to maintain this certification. We also mentor pharmacy students through collaboration with the University Of Montana School Of Pharmacy.
Our services include:
Reviewing medication orders and patient medication profiles prior to 1st dose administered, and acting in a formal consultant role to provide 24-hours per day, 7-days per week advice on any medication related issue. This improves patient safety and helps pharmacists and consultants to meet legal and JCAHO regulations. All of our clinical programs are linked to quality of care, cost savings and requirements of various regulatory agencies. Our emphasis is always in patient safety.
These programs include:
- Total TPN management, weekly nutritional assessments.
- Total management of aminoglycosides in adults, children and neonates.
- Total management of vancomycin.
- Total management of IV theophylline.
- Total management of heparin.
- Dosage adjustment of over 40 target medications cleared via the renal pathway.
- Multidisciplinary IV to PO switch program.
- Therapeutic substitutions to enhance formulary management.
- Proven coumadin dosing and monitoring protocol.
- Provide in-service education on any medication or hospital medication related subject.
- Assist in customizing and initiating QA activities relating to medication use, DUE (drug use evaluations), and DUR (drug use review).
- Assist in instituting an ADR (adverse drug reaction) surveillance program, including the necessary in-services to nursing personnel
